  • We request a referral and the last 3 chart notes from your primary care provider.
  • If we do not receive them before your appointment date, we may need to reschedule your appointment.
  • The documents can be faxed to 5033769201

To make your appointment as smooth as possible, please bring:

  • Valid Government-Issued Photo ID & Insurance Card.
  • Check in 45 minutes early if you’re a new patient & 15 minutes early for follow-ups to allow for paperwork needed.
  • Co-pay is due at the office visit
